Here could be a great place to start with a more comprehensive report. It could be called "Supplier Register" and it would be a simple list of all supplier in a big table. The list would contain "Supplier Name", "Supplier Approval Status", "Latest Risk Assessment", "Risk Assessment Date", "Approved Certifications", "Certification Expire", "Latest audit rating", "Audit conducted" etc. So that it would be a kind of massive table with just the most necessary bits of information in one report.
Also one report for each Supplier which shows a scorecard and all pertinent information.
